While you are responsible for the charges for the services rendered, we will file all insurance claims on your behalf, so please bring your insurances card(s) and photo ID with you. If you are a PPO or HMO patient, please check your provider listing to make sure that we are providers under your plan. If your insurance company requires a referral for your visit, please make sure that the referral is in place prior to your appointment.

Payment is required at the time of service, including co-pays, deductibles and services not covered by insurance. We accept personal checks, cash, Master Card and Visa as payment for services rendered
